Oddly enough when I watch Sherlock Holmes I get the munchies. There must be some link between Benedict Cumberbatch and food?

On a more serious note, your thoughts illustrate the role that entire swaths of the brain (and spinal cord) are involved in pain (perception, intensity, context, chronicity, etc.) and that finding a single diagnostic saying someone ‘is in pain’ or not is ludicrous (or at least laughable at this point). Treating chronic pain is also not just a simple “suck it up sunshine” or “just think positively and all will be positive.”
